Hora dourada: elaboração de procedimento operacional padrão para manejo de paciente pediátrico onco-hematológico febril

Abstract

The presente study aimed to develop a Standard Operating Procedure (SOP) to standardize the flow for the use and replenishment of the Golden Hour Box intended for the care of pediatric onco-hematology patients with suspected febrile neutropenia in a university hospital. This initiative arose from the institutional need to improve emergency care, ensuring the administration of empirical antibiotic therapy within 60 minutes of the patient’s admission, in accordance with the Golden Hour Protocol. This is na applied research, of a methodological and descriptive nature, developed from a literature review and analysis of the care routines in the pediatric ward and pediatric emergency department. The PDCA cycle guided all stages, including situational diagnosis, literature review, development, internal validation, and consolidation of the SOP. The analysis identified relevant gaps in the institutional flow, primarily related to the replacement of materials, the lack of standardization of fever criteria, and undefined responsibilities among members of the care team. The construction and internal review of the SOP and its supporting instruments, such as the box verification checklist and the protocol opening form, allowed to improvements the care pathway by ensuring greater operational clarity and strengthening the practical application of the protocol. The final document proved to be applicable, easy to understand, aligned with institutional routines, and capable of enhancing the quality of care provided to pediatric patients with febrile neutropenia. It is concluded that the SOP developed represents an important advancement in structuring and organizing care for pediatric onco-hematology patients with febrile neutropenia. It stands as an instrument that promotes greater patient safety, efficiency, and standardization of practices during the implementation of the Golden Hour Protocol.
