Summary: A biópsia renal é um procedimento eficaz quando se trata de diagnosticar doenças renais, estabelecer conduta terapêutica e prognóstico, no entanto, requer o mínimo de 5-20 glomérulos para adequação diagnóstica. Este estudo objetivou padronizar a técnica de biópsia renal felina sugerindo trajetória e calibre de agulha ideais, bem como região renal que maximizem o rendimento glomerular e minimizem complicações. Foram utilizados cinco cadáveres felinos sem alterações renais. Quatro foram submetidos a biópsia renal com agulhas Tru-Cut (14G, 16G e 18G) nos polos cranial, caudal e margem lateral através de trajetórias caudo-cranial e latero-medial (Grupo Tru-Cuts). Um animal teve os rins seccionados para análise virtual, que envolveu contagem de glomérulos por meio do software ImageJ (Grupo biópsia virtual), simulando amostras obtidas por biópsia com determinado calibre de agulha e região renal. A análise estatística incluiu testes de Friedman e Wilcoxon pareado para comparação entre calibres de agulhas, coeficiente de correlação intraclasse (CCI) para avaliação da reprodutibilidade e regressão logística com equações de estimação generalizadas (GEE) para complicações como transfixação das agulhas e fragmentação das amostras. Os resultados demonstraram hierarquia consistente de rendimento glomerular: 14G > 16G > 18G com diferença estatisticamente significativa entre todos os calibres. A trajetória caudo-cranial apresentou rendimento superior à latero-medial. A análise por polo renal revelou que a região lateral demonstrou médias consistentemente superiores para as agulhas 16G e 18G. O CCI indicou reprodutibilidade moderada a boa para a agulha 16G enquanto a 18G apresentou maior variabilidade. A agulha 16G demonstrou menor risco de fragmentação da amostra comparada à 18G, enquanto a 14G causou lesões macroscópicas renais em todas as punções. O modelo de biópsia virtual reproduziu o padrão hierárquico dos calibres, e superestimou o rendimento glomerular, indicando que fatores técnicos reduzem a eficiência da coleta. Concluiu-se que para felinos, a biópsia renal deve ser realizada na região lateral dos rins, com trajetória caudo-cranial, utilizando agulha 16G, para maximizar o rendimento glomerular e diminuir riscos de complicações.
Abstract: Renal biopsy is an effective procedure for diagnosing kidney diseases, guiding therapeutic decisions, and establishing prognosis; however, a minimum of 5–20 glomeruli is required for diagnostic adequacy. This study aimed to standardize the feline renal biopsy technique by identifying the ideal needle gauge and trajectory, as well as the renal region that maximizes glomerular yield while minimizing complications. Five feline cadavers without renal abnormalities were used. Four animals underwent renal biopsy using Tru-Cut needles (14G, 16G, and 18G) at the cranial pole, caudal pole, and lateral margin, following caudocranial and lateromedial trajectories (Tru-Cut group). One animal had its kidneys sectioned for virtual analysis, which involved glomerular counting using ImageJ software (virtual biopsy group), simulating biopsy samples obtained with specific needle gauges and renal regions. Statistical analysis included Friedman and paired Wilcoxon tests to compare needle gauges, intraclass correlation coefficient (ICC) to assess reproducibility, and logistic regression with generalized estimating equations (GEE) to evaluate complications such as needle transfixation and sample fragmentation. The results demonstrated a consistent hierarchy of glomerular yield: 14G > 16G > 18G, with statistically significant differences among all gauges. The caudocranial trajectory yielded a higher number of glomeruli than the lateromedial trajectory. Analysis by renal region showed that the lateral region consistently presented higher mean glomerular counts for the 16G and 18G needles. The ICC indicated moderate to good reproducibility for the 16G needle, whereas the 18G needle showed greater variability. The 16G needle was associated with a lower risk of sample fragmentation compared to the 18G, while the 14G needle caused macroscopic renal lesions in all punctures. The virtual biopsy model reproduced the hierarchical pattern among needle gauges but overestimated glomerular yield, indicating that technical factors reduce sampling efficiency. It was concluded that, in felines, renal biopsy should be performed in the lateral renal region, using a caudocranial trajectory and a 16G needle, in order to maximize glomerular yield and minimize the risk of complications.
