| Resumo: | Facial hematomas are accumulations of blood within soft tissues resulting from the rupture of blood vessels, generally caused by trauma or aesthetic procedures. Their natural resorption may take days or weeks, leading to significant aesthetic, functional, and psychosocial impairment. In this context, strategies capable of accelerating recovery while minimizing morbidity and patient discomfort have attracted increasing interest in aesthetic and dermatologic practice. Hyaluronidase is an enzyme that degrades hyaluronic acid in the extracellular matrix, reducing tissue viscosity and facilitating fluid diffusion. Although it is widely used in the management of complications associated with hyaluronic acid fillers, its off-label use in the treatment of hematomas has shown promising potential. The aim of this study is to reportthe use of hyaluronidase in the treatment of an extensive facial hematoma resulting from trauma. A 56-year-old female patient developed an extensive hematoma involving the temporal, zygomatic, and bilateral periorbital regions following facial trauma. Initial conservative treatment with a topical heparinoid ointment failed to produce significant improvement after eight days. The patient underwent serial subcutaneous injections of hyaluronidase diluted in saline solution at four-day intervals. Progressive regression of the hematoma was observed throughout the treatment, with complete resolution after 20 days, a high level of patient satisfaction, and no adverse effects. This case highlights the potential of hyaluronidase as a minimally invasive strategy to accelerate hematoma resolution in selected patients requiring rapid aesthetic recovery. Further studies are needed to better define indications, treatment protocols, and long-term outcomes. |
