Summary:	A radioterapia induz alterações químicas e mecânicas no ambiente bucal, afetando a composição das estruturas dentárias, glândulas salivares, mucosa, periodonto, ossos e todos os tecidos dentro do campo de radiação. A hipossalivação resultante da atrofia glandular, combinada com disfagia e disgeusia, cria um ambiente altamente favorável ao desenvolvimento de cáries relacionadas à radiação. A presença de recessão gengival e um fenótipo periodontal fino exacerbam essa condição, comprometendo a proteção dentária e prejudicando o controle do biofilme — particularmente quando combinada com uma prótese removível, que promove a retenção de biofilme dentário entre a prótese e as estruturas dentárias. A radioterapia para câncer de cabeça e pescoço compromete a integridade do periodonto, reduzindo a vascularização e induzindo acelularidade da membrana periodontal. O tratamento restaurador de lesões cariosas associadas à radiação deve ser integrado a uma abordagem multidisciplinar que inclua a preservação da saúde periodontal, a mitigação da hipofunção glandular, estratégias individualizadas de controle do biofilme e a inclusão do paciente em um programa preventivo adaptado ao seu perfil de risco específico. Quando a recessão gengival está presente, principalmente em pacientes com fenótipo periodontal fino, a intervenção cirúrgica torna-se essencial para aumentar a faixa de gengiva queratinizada, facilitar o controle eficaz da placa e promover a adaptação e o uso de próteses parciais removíveis. Este relato de caso descreve o tratamento clínico de um paciente submetido à radioterapia para câncer de cabeça e pescoço, apresentando cárie relacionada à radiação em dentes afetados por recessão gengival, diagnosticado através de exame clínico e radiografias, e edentulismo parcial inferior. A abordagem terapêutica incluiu a correção cirúrgica da recessão gengival por meio de enxerto gengival da região dos dentes 34 e 35 e 44 e 45, a restauração de lesões de cárie, reabilitação com próteses removíveis e inclusão do paciente em um programa de cuidados preventivos individualizado. Paciente foi acompanhado após a cirurgia e procedimentos restauradores, a fim de obter uma boa recuperação e melhora da qualidade de vida. Dada a escassez de literatura sobre o tema, este estudo tem como objetivo fornecer diretrizes clínicas para o manejo integrado em pacientes oncológicos submetidos à radioterapia.
Abstract:	Radiotherapy induces chemical and mechanical alterations in the oral environment, affecting the composition of dental structures, salivary glands, mucosa, periodontium, bones, and all tissues within the radiation field. Hyposalivation resulting from glandular atrophy, combined with dysphagia and dysgeus, creates a highly favorable environment for the development of radiation-related caries. The presence of gingival recession and a thin periodontal phenotype exacerbates this condition, compromising dental protection and impairing biofilm control— particularly when combined with a removable prosthesis, which promotes the retention of dental biofilm between the prosthesis and the dental structures. Radiotherapy for head and neck cancer compromises the integrity of the periodontium by reducing vascularization and inducing acellularity of the periodontal membrane. The restorative management of radiation￾ associated carious lesions must be integrated into a multidisciplinary approach that includes the preservation of periodontal health, mitigation of glandular hypofunction, individualized biofilm control strategies, and the inclusion of the patient in a preventive program tailored to their specific risk profile. When gingival recession is present, mainly in patients with the thin periodontal phenotype, surgical intervention becomes essential to increase the band of keratinized gingiva, facilitate effective plaque control and promote the adaptation and use of removable partial dentures. This case report describes the clinical treatment of a patient undergoing radiotherapy for head and neck cancer, presenting with radiation-related caries in teeth affected by gingival recession, diagnosed through clinical examination and radiographs, and partial lower edentulism. The therapeutic approach included surgical correction of the gingival recession using gingival grafting in the region of teeth 34 and 35 and 44 and 45, restoration of carious lesions, rehabilitation with removable prostheses, and inclusion of the patient in an individualized preventive care program. The patient was followed after surgery and restorative procedures to ensure a good recovery and improved quality of life. Given the scarcity of literature on the subject, this study aims to provide clinical guidelines for the integrated management of cancer patients undergoing radiotherapy.
