Summary: Problem statement New intraoral scanning systems (IOS) available in the dental market allow for the best image quality and fast scanning time. However, the accuracy of these IOS must be evaluated. Purpose The aim of this in vitro study was to evaluate the scanning time and marginal fit of computer aided design/ computer aided manufacturing (CAD/CAM) crowns obtained using different IOS (O1- Ominican 1.0, O2- Ominican 2.0, PS-Primescan). Material and methods A 3D-printed standardized composite resin die with a full-crown tooth preparation was scanned ten times with each IOS, followed by the design and milling of lithium disilicate ceramic crowns. The crowns were then fixed with a polyvinylsiloxane material on the die and scanned using micro-computed tomography to obtain the images used to measure the marginal fit of the crowns. Fifty-two measurements were performed for each image, and after analyzing the marginal misfit, the time of each scan was also evaluated. Data were analyzed for homogeneity, and one-way analysis of variance (ANOVA) and Tukey tests were performed (α=.05). Results The mean values (± standard deviation) for vertical misfit, were: O1- 46.7±16.4μm; O2- 33.8±21.4μm (p=.041); and PS-12.3±6.6μm (p<.001). The percentage of vertical misfits verified under 75μm were: O1- 76.5%; O2- 77.9%; and PS- 98.1%. The mean values (±SD) for horizontal misfit were: O1- 104.2±20.1μm; O2- 96.1±16.9μm; and PS- 89.0±14.2μm (p=.098). Mean scanning times (±SD) were: O1- 37.4±3.1s; O2- 34.8±2.7s; PS- 27.8±1.9s; with similar time for O1 and O2, which differed from PS. Conclusions Significant differences were observed in the scanning time and marginal fit of the CAD/CAM ceramic crowns obtained using distinct IOS, with PS presenting the best results. Hardware and software enhancements in IOS play a pivotal role in influencing these results. Clinical Implications The Primescan IOS system provided ceramic crowns with more accurate marginal fit, in addition to shorter scanning times as compared to Omnicam 1.0 and Omnicam 2.0 systems when using the same milling unit.
