Infiltração de Hialuronato de sódio no tratamento de disfunção temporomandibular articular e artralgia: relato de caso clínico

Abstract

Viscosupplementation of the temporomandibular joint (TMJ) is indicated for the control of arthralgia and temporomandibular disorders of joint origin: displacement of the TMJ articular disc with or without reduction and degenerative TMJ diseases such as osteoarthritis and osteoarthrosis. The aim of this study was to carry out a qualitative study of narrative review of the scientific literature on the use of sodium hyaluronate (SH) in the treatment of temporomandibular disorder (TMD) and arthralgia associated with the clinical case report. The search strategy included clinical searches without excluding the type of study, covering the period from 2005 to 2020. For the bibliographic search, PubMed / Medline, SciELO and CAPES Periodicals were used, with the following descriptors in portuguese: Temporomandibular Joint or Temporomandibular Disfunction, viscosupplementation and sodium hyaluronate, as well as the terms in English temporomandibular joint disorder, injections and intra-articular, viscosupplementation and hyaluronic acid. 289 articles were identified in PubMed / Medline, 6 articles in SciELO and 290 articles in the CAPES Journals portal. According to the established inclusion and exclusion criteria, 28 articles were included, which were read by two reviewers. In addition, a clinical case report of a 39-year-old female patient with complaints of arthralgia in the left TMJ, limited mouth opening, and constant headache is presented. The procedure of viscosupplementation of the left TMJ was performed, injecting 1.0mL of Osteonil into the joint capsule in the upper compartment. The patient reported immediate improvement in pain, confirmed 15 and 120 days after viscosupplementation. The reviewed scientific literature reported clinical success in using viscosupplementation with sodium hyaluronate in the treatment of joint TMD and arthralgia. The treatment protocol used proved to be effective in controlling pain and recovering the biomechanics of the temporomandibular joint.
