| Summary: | O presente estudo teve como objetivo avaliar o uso isolado e combinado do gel à base de flúor/sílica/cálcio e da terapia a laser de baixa intensidade (LBI) no manejo da hipersensibilidade dentinária (HD). Foram incluídos 45 pacientes com HD que apresentavam pelo menos um dente com dentina cervical exposta na superfície vestibular. A presença de HD foi confirmada por uma resposta positiva ≥ 40 mm na escala visual analógica (VAS) após a aplicação do jato de ar e de água. Os participantes foram randomizados em três grupos: FSC (gel à base de flúor, sílica e cálcio), LBI (laser de baixa intensidade) e FSC/LBI (combinação das terapias). Os grupos FSC e FSC/LBI receberam três aplicações do gel em consultório, com intervalos de 72 horas, associadas ao uso domiciliar do gel à base de flúor, sílica e cálcio por dois meses, seguido da substituição por um gel à base de flúor e sílica (sem o booster de cálcio) até completar três meses. Os grupos LBI e FSC/LBI receberam aplicações de LBI nas três sessões iniciais e também nas consultas de acompanhamento aos 7 dias, 15 dias, 1 mês e 3 meses. A dor foi avaliada após estímulos evaporativos e táteis, sendo quantificada por meio da VAS e da escala de Schiff no início do estudo, imediatamente após cada sessão e nos acompanhamentos de 7, 15, 30 dias e 3 meses. A qualidade de vida foi avaliada utilizando o questionário de experiência de HD (DHEQ-15) no início do estudo e durante o acompanhamento. Todos os grupos apresentaram redução imediata da dor em resposta ao estímulo evaporativo após a primeira intervenção, sem diferenças significativas entre os tratamentos. Um padrão semelhante foi observado na segunda e última sessão. Em comparação ao início do estudo, todos os grupos demonstraram redução gradual da dor ao longo do tempo. Para as avaliações táteis e pela escala de Schiff, apenas o grupo LBI apresentou redução significativa após as três primeiras sessões, enquanto todos os grupos mostraram reduções significativas ao longo do tempo. A qualidade de vida melhorou em todos os grupos. Conclui-se que o gel FSC e o LBI foram eficazes na redução da dor e na melhora da qualidade de vida de pacientes com HD. Entretanto, a associação dessas terapias não proporcionou benefícios adicionais em comparação ao uso isolado de cada abordagem, sugerindo eficácia clínica semelhante entre os tratamentos. |
| Abstract: | The present study aimed to evaluate the isolated and combined effects of a fluoride/silica/calcium (FSC) gel and low-level laser therapy (LLLT) in the management of dentin hypersensitivity (DH). Forty-five patients with DH presenting at least one tooth with exposed cervical dentin on the buccal surface were included. DH was confirmed by a positive response of ≥40 mm on the Visual Analog Scale (VAS) following air and water stimulation. Participants were randomly allocated to three groups: FSC (fluoride/silica/calcium gel), LLLT (low-level laser therapy), and FSC/LLLT (combined therapy). The FSC and FSC/LLLT groups received three in-office applications of the FSC gel at 72-hour intervals, combined with home use of the gel for two months, followed by replacement with a fluoride/silica gel (without the calcium booster) for three months. The LLLT and FSC/LLLT groups received laser applications during the first three treatment sessions and at follow-up visits after 7 days, 15 days, 1 month, and 3 months. Pain was assessed following evaporative and tactile stimuli using the VAS and the Schiff Cold Air Sensitivity Scale at baseline, immediately after each treatment session, and at the 7-day, 15-day, 1-month, and 3-month follow-up visits. Oral health-related quality of life was assessed using the Dentine Hypersensitivity Experience Questionnaire (DHEQ-15) at baseline and throughout the follow-up period. All groups exhibited an immediate reduction in pain elicited by evaporative stimulation after the first intervention, with no significant differences among treatments. A similar pattern was observed after the second and third treatment sessions. Compared with baseline, all groups showed a gradual reduction in pain over time. For tactile sensitivity and Schiff scale assessments, only the LLLT group demonstrated significant reductions immediately after the first three treatment sessions, whereas all groups exhibited significant improvements over time. Oral health-related quality of life also improved in all groups. It can be concluded that both the FSC gel and LLLT were effective in reducing pain and improving the quality of life of patients with DH. However, the combination of these therapies did not provide additional benefits compared with either treatment alone, suggesting similar clinical efficacy among the treatment approaches. |
