| Resumen: | Background: Real-world data on the impact of video monitoring on heart failure (HF) prognosis are limited and inconclusive. Objectives: This article describes a randomized clinical trial protocol designed to evaluate the efficacy of a video monitoring and education strategy in patients with heart failure with reduced ejection fraction (HFrEF). Methods: This is a randomized, parallel group trial with blinded outcome assessment. Eligible patients will be invited to participate during hospitalization. Data collection will include sociodemographic and clinical variables, laboratory test results, current medications, and cardiovascular physical examination. Validated instruments will be used to assess clinical congestion score, European HF self-care, HF knowledge, treatment adherence, quality of life, and cardiorespiratory fitness. After discharge, the control group (CG) will receive usual care, while the intervention group (IG) will have their clinical discharge summary shared with primary healthcare providers (nurse and physician) to discuss transitional care. IG participants will also receive specialized cardiovascular nursing appointments through video monitoring at 7, 30, 60, and 180 days. The primary outcome is self-care score, while secondary outcomes include quality of life, HF knowledge, treatment adherence, cardiorespiratory fitness, mortality, and hospital readmissions. Results: The protocol will begin in May 2025 and is expected to complete patient recruitment by May 2026. A total of 140 patients are expected to be included, with 70 in each group. This research is funded by National Council for Scientific and Technological Development (CNPq), Brazil, from January 2025 to January 2026. In contrast to strategies based on mobile devices or voice telemonitoring, video monitoring hypothetically creates greater trust between patients and professionals, improving measured outcomes. Conclusions: This study highlights the use of video monitoring in heart failure management, improving nurse-patient relationships, self-care, and treatment adherence. It emphasizes the role of nurses in transitional care and education, reducing readmissions and mortality. |
