Título:	Cirurgia Ortognática como Tratamento para a Síndrome da Apneia Obstrutiva do Sono: Relato de Caso

Resumo:	Orthognathic surgery is indicated not only to correct dentofacial malocclusion and enhance facial aesthetics but also as an alternative to CPAP in the treatment of severe Obstructive Sleep Apnea Syndrome (OSAS). OSAS is characterized by intermittent obstructions of the upper airways during sleep, leading to desaturation and sleep pattern disturbances, which compromise patients' quality of life. It is diagnosed through polysomnography. When properly indicated, conventional ortho-surgical treatment emerges as an effective therapeutic option for individuals with severe OSAS. Additionally, the surgery-first approach has shown excellent patient satisfaction due to the immediate resolution of OSAS. This study presents the case of a 35-year-old patient with a Class II facial profile and Angle Class I occlusion, diagnosed with severe OSAS, who was treated with bimaxillary orthognathic surgery and mentoplasty using the Surgery-First technique. In the postoperative period, complete resolution of OSAS and a significant improvement in sleep patterns were observed. The proper indication of orthognathic surgery in OSAS patients has shown a positive impact on sleep quality and, consequently, on quality of life.
