Title: Síndrome cardiorrenal tipo 2 em cães: estudo retrospectivo

Summary: O eixo cardíaco-renal possui forte correlação, sendo que a disfunção em um desses órgãos pode afetar diretamente o outro, de forma crônica ou aguda. Essa ligação fisiopatológica que pode ocorrer é definida como a síndrome cardiorrenal, e pode ser subdividida em cinco grupos, de acordo com a origem da enfermidade, são eles: síndrome cardiorrenal aguda e crônica, síndrome renocárdica aguda e crônica e a síndrome secundaria. Em Medicina Veterinária o acometimento dessa enfermidade, na sua forma cardiorrenal crônica, é prevalente em cães que apresentem distúrbios na valva mitral, e pode-se relacionar a sua ocorrência com o uso de medicamentos, como os diuréticos de alça, comumente prescritos em cães com a doença mixomatosa da valva mitral. Visto a falta de uma definição exata da ocorrência da fisiopatologia da síndrome e seu diagnostico possuir uma natureza complexa, o presente estudo visa analisar a ocorrência de cães que apresentem a doença em valva mitral de forma crônica e posteriormente demonstrem sinais de acometimento da função renal, a partir do estudo destes casos que ocorreram no Hospital Veterinário da Universidade Federal de Uberlândia, no período de 2021 a 2025. Foram avaliados 400 cães nesse período, sendo 91 se encaixando em critérios de inclusão e exclusão, dentre os quais, 10 apresentaram desenvolvimento da SCR tipo 2. Foi realizado o teste de Fischer, que não apresentou significância entre a SCR2 e o óbito do animal (p = 0,087), entretanto ao se avaliar a probabilidade de desenvolvimento de edema, o desenvolvimento da SCR2 teve significância nos valores (p=0,033), possivelmente relacionada ao uso e dosagem de diuréticos de alça como tratamento desses pacientes. Com base nesses resultados, a ocorrência de SRC tipo 2 foi de 10,98% dos cães com DMVM.
Abstract: The cardiac-renal axis shows a strong correlation, in which dysfunction of one of these organs can directly affect the other, either chronically or acutely. This pathophysiological connection is defined as cardiorenal syndrome, which can be subdivided into five groups according to the primary origin of the disease: acute cardiorenal syndrome, chronic cardiorenal syndrome, acute renocardiac syndrome, chronic renocardiac syndrome, and secondary syndrome. In Veterinary Medicine, the chronic cardiorenal form of this condition is particularly prevalent in dogs with mitral valve disorders, and its occurrence can be associated with the use of medications, such as loop diuretics, which are commonly administered to dogs affected by myxomatous mitral valve disease. Considering the lack of a precise definition of the syndrome’s pathophysiology and the complex nature of its diagnosis, the present study aims to analyze the incidence of dogs diagnosed with mitral valve disease that subsequently develop signs of renal dysfunction, based on cases recorded at the Veterinary Hospital of the Federal University of Uberlândia between 2021 and 2025. A total of 400 dogs were evaluated during this period, of which 91 met the previously established inclusion and exclusion criteria. Among these, 10 developed type 2 CRS. Fisher’s test was performed and did not show a significant association between CRS type 2 and death of the animal (p = 0.087). However, when evaluating the probability of edema development, the occurrence of CRS type 2 showed statistical significance (p = 0.0033), possibly related to the use and dosage of loop diuretics as part of the treatment for these patients. Based on these results, the occurrence of type 2 CRS was 10.98% among dogs with DMVM.
