Conciliação medicamentosa conduzida por farmacêutico clínico no departamento de urgência e emergência: revisão de literatura e criação de um manual institucional

Abstract

The patient's safety involves several mechanisms, including safe prescription, thus avoiding medication errors. These errors can be reduced with medication conciliation, which should be performed during the transition of care and can be conducted by any health professional. However, studies have shown that the health professional who has easiest and most promising results during this process is the clinical pharmacist. Thus, the objective of this study was to perform a literature review on medication conciliation in the emergency department conducted by the clinical pharmacist, as well as to create an institutional manual for the Hospital de Clínicas da Universidade Federal de Uberlândia (HC-UFU/ EBSERH). For the literature review, searches were performed using descriptors in health sciences in both Portuguese and English, with Boolean operators in the chronological period of 15 years, using primary sources and also gray literature. For the creation of the institutional manual, a model already standardized by EBSERH was used, which will then be presented as a product of the residency completion work. For the final approval of the manual, a meeting will be held with the quorum of pharmacists from HC- UFU/EBSERH to make final suggestions, and then, after making the necessary changes, it will be sent to the Quality Management Unit/HC-UFU/EBSERH, and thus approved for institutional use.
