Reposicionamento labial e otimização estética: relato de caso clínico

Abstract

In view of the growing social concern with favorable aesthetics, the dental surgeon has been led to comprehend the needs of patients in a comprehensive way to establish in the dental treatment beyond the physiological standard, a harmonious aesthetic relationship. This is considered by the satisfactory association between teeth, lip structure and gingival exposure, the last one being classified as excessive when above 3mm, characterizing the “Gingival smile”. In different etiologies, several surgical modalities have been reported to eliminate unwanted gingival exposure, sometimes associated, and in this context, Lip Repositioning appears as a conservative, reversible surgery, with a great prognosis. The aim of this studie is to present a clinical case in which the Lip Repositioning surgery was performed, among other cosmetic dental procedures, as part of an integrated planning. Report of case: 23 year-old female patient, presented gingival exposure above 5 mm and lip hyperactivity when smiling, short anterior clinical crowns with white fluorosis spots, bilateral maxillary bony prominences and marginal insertion of the labial frenulum. Procedures that occurred in two surgical stages were indicated, the first with labial frenectomy and flap surgery with osteoplasty of the posterior maxillary vestibular region; and the second one with Lip Repositioning and Gingivoplasty, occurring without complications. After that, methods of adjusting the color of the teeth were performed with a session of Microabrasion and home teeth whitening. It is concluded that the success of aesthetic rehabilitation is related to the execution of a multidisciplinary treatment plan, capable of establishing harmony between all the smile components. After completion and healing of the case, accompanied for up to 1 year, the effectiveness of the techniques employed was demonstrated, adequate gingival exposure, gain in lip volume, improvement of the aesthetics of smile and the patient satisfaction.
