Substituição de prótese do tipo protocolo para overdenture em paciente geriátrico: relato de caso

Abstract

Edentulism is considered a health problem in Brazil, because it affects the patient by causing psychic, social and functional problems. The prosthesis implant treatment should be planned and executed in order to have a correct maintenance and longevity. Therefore, the present study shows, through clinic case report, the substitution of a Branemark prosthesis to an Overdenture prosthesis, considering the patient’s bio psychosocial principles. A 92 year old female patient participated in the Overdenture extension project which was developed for totally toothless people on Health Technical School Geriatrics (ESTES UFU) and she complained about halitosis, difficulty in cleaning the prothesis and general discomfort when wearing it. The intraoral clinical examination revealed a mucosa-supported upper prosthesis and a fixed prosthesis which was the kind of a lower implant protocol. That showed inappropriated occlusal estability, a high degree of wear of the resin teeth and the presence of a biofilm, which demonstrated the patient inability to make the correct tooth hygiene of the protocol takimg into consideration the loss of motor coordination as the result of the patient’s age. The treatment plan was defined by adequating the buccal oral environment, toothbrushing instructions and, afterwards, the construction of two new prostheses. It was developed an upper conventional prosthesis and a lower overdenture prosthesis on equator pillars. It took advantage of four implants on a correct position in the protocol. After the installation, it was possible to diagnose a better masticatory comfort and a better cleaning of the prothesis. Thus, it was promoted quality and longevity in treatment accordding to the necessity and age of the patient and it was possible to bring back her oral health and self-steem.
