    <summary type="text">Title: Estudo da flora vaginal em homens transgênero em uso de testosterona
Abstract: Transgender men who retain female genital structures may have specific gynecological care&#xD;
needs, especially when using testosterone hormone therapy. Prolonged exposure to high levels&#xD;
of testosterone and estrogen suppression promotes changes in the vaginal epithelium and local&#xD;
microbiota, potentially favoring dysbiosis, such as bacterial vaginosis (BV). Despite this, there&#xD;
is still a scarcity of studies evaluating these changes in this group of patients. The objective of&#xD;
this study was to analyze the changes in the vaginal microbiota and the prevalence of bacterial&#xD;
vaginosis in transgender men using testosterone hormone therapy, through microbiological&#xD;
analysis of vaginal smear slides and the Nugent score, correlating the findings with clinical&#xD;
characteristics and hormone use. This is an observational, cross-sectional study conducted with&#xD;
11 transgender men attending a specialized outpatient clinic. Vaginal secretion samples were&#xD;
collected for Gram staining analysis, with microbiota classification according to the Nugent&#xD;
score, in addition to the application of a clinical questionnaire on the characteristics of vaginal&#xD;
discharge. The average age of the participants was 29 ± 4.59 years, with 73% currently using&#xD;
testosterone. Lactobacillus was absent in 82% of the samples, and numerous Gram-variable&#xD;
bacilli were present in 36% of cases. The average Nugent score was 5.82 ± 2.86, indicating a&#xD;
predominance of intermediate microbiota, with cases compatible with bacterial vaginosis. Only&#xD;
27% of participants reported vaginal discharge with clinical alterations. No significant&#xD;
association was observed between testosterone use and microbiological or clinical findings.&#xD;
However, the presence of Gram-variable bacilli showed a significant association with higher&#xD;
Nugent scores, compatible with bacterial vaginosis. It is concluded that transgender men using&#xD;
testosterone present relevant alterations in the vaginal microbiota, characterized by a reduction&#xD;
in Lactobacillus and a greater presence of microorganisms associated with bacterial vaginosis.&#xD;
These findings reinforce the need for specific attention to the health of this population, as well&#xD;
as the performance of studies with larger samples for a better understanding of the clinical&#xD;
implications of these alterations.</summary>

    <title>Mucormicose rinocerebral: relato de caso clínico</title>

    <summary type="text">Title: Mucormicose rinocerebral: relato de caso clínico
Abstract: Mucormycosis is a rare, invasive, and highly lethal fungal infection caused by fungi of the order Mucorales. This study reports the clinical case of a 65-year-old male, a construction worker with hypertension, who sought medical care presenting with progressive headache, hemifacial paresthesia, and lesions on the palate and nostril. The patient had a history of long-term high-dose corticosteroid use and a recent diagnosis of Type 2 Diabetes Mellitus. Imaging exams revealed paranasal sinus opacification and bone erosions extending to the right frontal lobe, consistent with a cerebral abscess. The diagnosis of Mucor SPP was confirmed through incisional biopsy for culture. Therapeutic management involved the administration of a cumulative dose of 9300 mg of liposomal Amphotericin B, with rigorous monitoring of renal function, followed by consolidation therapy with Isavuconazole. The clinical outcome was favorable, showing regression of inflammatory signs and regular outpatient follow-up. This case reinforces the imperative need for early diagnosis and a multidisciplinary approach given the aggressiveness of rhinocerebral mucormycosis.</summary>

    <title>Doenças Raras e Saúde Pública: dados do serviço de Genética Médica em um Hospital Universitário Federal</title>

    <summary type="text">Title: Doenças Raras e Saúde Pública: dados do serviço de Genética Médica em um Hospital Universitário Federal
Abstract: This descriptive study aimed to analyze outpatient genetics medical attendances conducted at the the Clinical Hospital of the Federal University of  Uberlandia  (HC-UFU) from 2003 to 2022, focusing on conditions of genetic origin classified as rare diseases. Patients with Down syndrome and those without a confirmed genetic diagnosis were excluded. The analysis involved data collected from institutional records and categorized into four lines of care: inborn errors of metabolism, congenital anomalies, genetic counseling, and neurogenetics. The results demonstrated progressive growth in the number of attendances over the period, despite punctual interruptions, such as those occurring between 2013 and 2015, and a decline in 2020 due to the COVID-19 pandemic. The peak in attendances occurred in 2017. The structuring of attendances into specific lines of care starting in 2011 reflected greater organization and alignment with the National Policy for Comprehensive Care for People with Rare Diseases. The discussion highlights the main challenges faced in the diagnosis and treatment of these conditions, such as resource scarcity, regional inequalities, and difficulties in incorporating high-cost therapies into the SUS, as well as the increase in health judicialization. The study underscores the importance of HC-UFU as an emerging reference center for rare disease care and the need for more equitable and sustainable public policies. In conclusion, the systematization and continuous analysis of data are essential for planning and improving services, while also providing subsidies for future research and the formulation of strategies to expand access to early diagnosis and appropriate treatments. This work fills a gap in the national literature by presenting longitudinal quantitative data on genetics medical attendances in a federal public hospital located in the interior of the country.</summary>
